Valley West Care Center Deploys Innovative Disease Management System to Enhance Resident Quality of Care

Riverside Health Care, a leading long-term care provider, has selected COMS Interactive, LLC, to enhance its skilled nursing facilities’ clinical care programs. The COMS (Clinical Outcomes Management Solutions) flagship product, Daylight IQ™, is a core component of Riverside Health Care’s disease management program. The installation of Daylight IQ™ has been completed at Delta Rehabilitation & Care Center, in Lodi, CA; Paradise Skilled Nursing, in Paradise, CA; and at Valley West Care Center, located in Williams, CA.

The only true disease management product on the market, the Daylight IQ™ Software as a Service (SaaS) product features a series of integrated, disease-based clinical protocols that significantly empower the entire care team by identifying changes in condition at the bedside. Daylight IQ™ highlights abnormal findings and provides the nursing team with appropriate interventions and/or physician communication. Early detection of such changes allows the care team to quickly address/remedy potential additional health challenges.

The Daylight IQ™ product suite focuses upon improving quality of care by closely tracking changes in condition and initiating appropriate clinical interventions. Key features of the product suite include an Automated Disease Care Guide Library, a Dynamic Nursing Assessment and Compare Module, all of which work in conjunction with each resident’s disease profile. An important component of the Daylight IQ™ program is reducing unnecessary hospital readmissions by more than 50%.
