CRMC Providing Transitional Care Services

 

The Colusa Regional Medical Center (CRMC) has announced the recent partnership with area medical facilities to provide transitional care services.

“We are underutilizing our capacity and by partnering with neighboring hospitals, we can offer a valuable service to our community,” said CRMC CEO Walt Beck.

Transitional Care is the coordination and continuity of health care of a patient during a movement from the healthcare setting to either another facility of their home.

“Some patients have complex needs or suffer from chronic or acute illnesses that still require a hospital atmosphere,” said CRMC Physician Dr. Gregory Burt, “and in most of the cases it would do the patient more harm to send them home, but a nursing facility isn’t needed.”

As more skilled nursing facilities are impacted with long-term care patients, there will be fewer beds for recovery.

“We have the appropriate medical services available to the patient when in transitional care,” said Burt.

CRMC has a physician on staff and can provide rehabilitation services, radiology, and lab services when needed, Beck commented.

“The goal is to release the patient back to their homes as safely as possible,” said Beck.

Since health care has changed over the last several years, Beck commented that CRMC is finding a way to provide services to the community.

“We are trying to figure out what the hospital can market with what we have available,” he said.

Katherine Hughes, Director of Nursing Administration at Colusa Regional Medical Center commented that since its implementation the hospital has averaged 2-3 transfers per day for transitional care services.

“This is a win-win situation for the hospital, we can fill beds that otherwise empty while providing a needed service the community,” said Beck.
